Contacting Your Care Team

Scheduling a Visit

To receive services at a Piedmont Health community health center, you must register as a new patient. To do that, call the center closest to you to make a new-patient appointment with a primary care provider (PCP). You may find a complete list of our centers, PCP and hours on the Locations pages.

Existing Patients

If you are an existing patient, please contact your PCP’s care team to schedule or coordinate an appointment at your health center. Your Care Team includes the PCP (this might be a doctor, nurse practitioner or physician’s assistant), other providers that work with your PCP, nurses, medical assistants and front desk staff.

Scheduling a Same Day Appointment

Piedmont Health’s centers offer same day appointments. If this is an urgent problem, please call the center to make an appointment for that day.

24/7 Care: After Hours Care

If you have a medical emergency, please call 911 or go to nearest emergency department. If you have a non-emergency healthcare concern, please call the health center during office hours and when our medical offices are closed contact UNC Health Links, our answering service (1-888-267-3675). These trained RNs will be able to help address your basic questions. The nurse will contact the designated provider on-call if needed.
