My First Visit

What to bring for your first visit?

Welcome to Piedmont Health! We’re proud you chose us as your healthcare home. Our focus is to give our community access to quality care and help you manage the health and well-being of you and your family. We’re here to serve you with the attention you deserve.

On the day of your first appointment, please arrive 15 minutes early to register. In order to register you in a timely manner, please bring the following:

A valid ID such as a:

  • Driver’s license
  • Employment or school ID
  • Social Security Card
  • Green Card
  • Valid Passport
  • Veteran’s ID
  • Medical Card or Insurance Card (If you have one). We accept most health insurance plans, including Medicaid and Medicare

If you are uninsured, payment for services is based on a sliding-fee scale according to family income. If you do not have insurance, please bring:

1. Proof of income, for every household member. This may include:
• a paycheck stub or
• a copy of your tax form reporting your income or
• a bank statement if you receive direct deposit

2. Proof of residence. For example, a bill addressed to you sent by mail.
3. Evidence of health or dental insurance coverage, if any.
4. A Picture I.D. card

Scheduling a New Patient Visit

To receive services at a Piedmont Health Community Health Center, you must register as a new patient. To do that, call the center closest to you to make a new-patient appointment. You may find a complete list of our centers on the Locations page.

During Your First Visit

We want to optimize your first time that you visit with the care team. The Care team will want to understand your complete medical history. If you are transferring from other provider or recently seen another medical provider, including a specialist, or have been seen in the emergency room or hospital, please bring any test results, care plan or discharge summary that you received. This is important information we want to add to your medical history. If you want us contact the provider to transfer your records to our practice please ask the operator to connect you to medical record department when you call. It is also important to bring all of your medications (prescriptions and over the counter medicines) to this first appointment.  Optimal health will include our Care Team coordinating your care. You will have a medical home with us!
