PHS Announces Closure of Sylvan Site, Transition of Care

CHAPEL HILL, NC — July 1, 2026 — Piedmont Health Services (PHS) today announced the upcoming closure of its Sylvan Community Health Center site, located at Sylvan School in Snow Camp, effective July 31, 2026. The decision comes as part of a strategic effort to consolidate resources and enhance the delivery of comprehensive care across its broader regional network.

To ensure continuity of care, Robin Whitten, PA, a deeply respected provider at the Sylvan location, will be transitioning to the Siler Community Health Center team. Located at 224 South 10th Avenue in Siler City, the Siler site will welcome Whitten’s patients. Patients can also transfer to our established teams in Burlington, Moncure, and Sanford.

Continuity of Care and Medical Records

All patient medical records will remain securely stored within the Piedmont Health Services network. Current Sylvan patients can seamlessly transition their care to the Siler City location or any other convenient PHS site. Ongoing treatments and appointments will continue as scheduled at the Sylvan location through July 30, 2026.

Managed NC Medicaid Guidelines

Patients enrolled in a Managed NC Medicaid plan (including Healthy Blue, Carolina Complete Health, AmeriHealth Caritas, or UnitedHealthcare) who currently have Sylvan Community Health Center listed as their designated medical home can continue receiving care with Piedmont Health Services. However, these patients must officially update their primary care site to their new preferred location (e.g., Siler City, Moncure, or Sanford).

Piedmont Health staff members are fully equipped to assist patients with this site-change process during their transition. Patients may also contact their Medicaid plan directly using the phone number listed on the back of their insurance card.

Support and Appointment Scheduling

PHS staff members are actively working to support patients through every step of this transition. For questions, concerns, or to schedule future appointments at an alternative Piedmont Health location, community members can reach out via the following contact methods:
